Position Summary:
To provide nutrition education, counseling and interventions as appropriate to End Stage Renal Disease (ESRD) patients and authorized caregivers in clinical settings to maximize the patient’s nutritional well-being.

Essential Duties and Responsibilities:
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive
1. Independently evaluates, develops and completes comprehensive nutrition assessment and reassessment consistent with CMS regulations.
2. Works with patient and IDT to establish, document and update patient’s plan of care (POC).
3. Assesses ongoing nutritional status to determine appropriate nutrition interventions and recommendations. Consults with LTC facility staff (RN, RD) and physicians regarding lab tests and medications adjustments as needed.
4. Develops individualized Medical Nutrition Therapy for patients based on reported lab results and individualized goals.
5. May request assistance from the lead dietitian as needed with advanced or complex clinical renal nutrition situations.
6. Provides nutrition education and counseling based on individualized patient needs including nutrient requirements, knowledge and abilities, medical, economic and social situation.
7. Selects and develops education materials and methods appropriate for individual patient.
8. Monitors nutritional status, laboratory values, dialysis adequacy, adherence to diet and response to nutrition related medications and therapy to determine appropriate nutrition intervention and recommendations.
9. Documents nutrition progress notes at least monthly.
10.Conduct nutrition assessment upon patient admission, 90-day assessment and annually.
11.Review monthly lab results with each patient and/or LTC facility and provide patient/LTC facility with printed patient nutrition report.
12.Evaluates outcomes and identifies factors that may contribute to undesirable results and documents appropriately.
13.Participates in patient care conferences at LTC facility, continuous quality improvement meetings and departmental and interdepartmental meetings as needed.
14.Travel to assigned facilities and assists in coverage of Nutrition Services in cases of LOA, vacations and census increase.
15.Responsible for self-professional development and maintenance of necessary credentials.

Minimum Qualifications:
Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required.
1. Bachelor’s degree in Nutrition, Dietetics, or similar area required.
2. Registered Dietitian with at least 12 months’ clinical experience as required by CMS.
3. At least 1 year specialized renal experience.
4. State licensure or certification required if licensure or certification is available in the state where the facility is located.
5. Intermediate computer skills and proficiency with MS Word, Excel, Outlook required as well as functional proficiency with Concerto specific applications within 60 days of employment.

Language Skills / Visual Requirements:
Fluent in the written and verbal skills necessary to perform successfully the essential functions, duties and responsibilities of the position. Vision adequate to perform essential duties and responsibilities of position.

Physical Demands:
1. Ability to lift minimum of 5 pounds to a maximum of 35 pounds unassisted; able to sit for extended periods of time with some standing, stooping, walking stretching, reaching, lifting; moderate range of body motions.
2. Ability to hear and interact by phone and in person clearly and efficiently.

Work Environment:
Job required tasks normally do not involve exposure to blood, body fluids, or tissues; however, exposure or potential exposure may be required as a condition of employment and appropriate personal protection measures are required in those instances. Normal hazards are associated with business travel.
