QUALIFICATIONS:

1. Holds a Masters Degree from a graduate school of Social Work accredited by the Council on Social Work Education
2. Licensed by the State of Illinois
3. LCSW preferred

POSITION SUMMARY:


The social worker provides social services to patients and their families that are directed at supporting and maximizing the social functioning and adjustment of the patient. Responsible for conducting psychosocial evaluations, participating in team review of patient progress and recommending changes in treatment based on the patient’s current psychosocial needs; providing for casework or group work services to patients and their families in dealing with the special problems associated with ESRD; and identifying community social agencies and other resources and assisting patients and families to utilize them. Provides consultative services for LTC facilities with referred dialysis patients.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

Must be able to handle light material (20#). Requires close mental, visual and auditory attention to patient/families. Work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious disease/materials.

JOB FUNCTIONS/EVALUATION CRITERIA:

 Provides excellent customer service as it applies to all external and internal customers including patients, staff and physicians
 Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner
 Demonstrates respect to individual an cultural beliefs and lifestyles of patient/s families and co-workers
 Follows all pertinent policies and procedures. Assists in the development of procedures related to social welfare of the renal patient
 Assesses needs and resources and provides interventions appropriate for age, socioeconomic status, and physical capabilities of patient
 Assists all patients to obtain necessary funding for dialysis treatments
 Assists patients with travel dialysis arrangements when needed

Policy and Procedure Manual:

 Assists with patient transfer arrangements when needed
 Assists with patient transportation needs to and from dialysis when needed
 Demonstrates full working knowledge of and refers as needed to community resources
 Works collaboratively as a member of the multidisciplinary patient care team and actively participates in ‘Team Conferences’ and the development, implementation, and evaluation of Interdisciplinary Plan of Care and Comprehensive Interdisciplinary Assessment
 Actively participates in QAPI
 Completes all necessary documentation
 Serves as a social service resource for all patients and families, and staff.
 Identifies and properly intervenes and reports suspected cases of abuse and neglect
 Actively participate in ongoing education opportunities and share relevant information with the renal team to enhance team skill in working with end-stage renal disease patients
 Responsible for maintaining continuing education hours and state licensure regulations where applicable
 Assures regulatory compliance with all social work issues
 Communicates professionally and serves as a positive role model with staff, physicians, administration, patients, families and community
 Maintains a clean and safe work environment
 Other duties as assigned
