ANEMIA MANAGEMENT / ACCESS MANAGEMENT / QAPI RN CLINICAL COORDINATOR

Focus: Anemia Management, Access Management, QAPI
Status: Full Time Position
Reports To: Regional Directors of Operations
Qualifications:
1. Graduate of an accredited School of Nursing
2. Currently licensed as a Registered Nurse in the State of Illinois
3. Previous dialysis experience and/or Nephrology Certification preferred

Physical Demands and Work Environment:
This is primarily an in-office position, but on occasion may be required to educate RN’s in unit and travel to the dialysis units. Employee is required to report to the corporate office and work full-time hours. Physical demands are minimal and employee may be required to lift no more than 15-20 lbs at one time unless employee must work as a dialysis RN in a unit then will be required to follow the RN Job Description.

RN Clinical Coordinator Job Duties / Functions:
1. All duties as RN Clinical Coordinator are performed under the direction of and in cooperation with the Medical Director and/or Nephrologist and in accordance with Physician orders, Concerto Policies and Procedures, Concerto Protocols, CMS, KDOQI Guidelines, all State and Federal Regulations, and within the scope of the Illinois Nurse Practice Act
2. Provides excellent customer service as it applies to all external and internal customers including patients, staff and physicians 3. Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner
4. Utilizes assessment information, patient learning needs and age appropriate considerations to develop and/or contribute to a plan of care that will reduce complications and promote health.
5. Evaluates interventions, documents, and implements changes to plan collaborating with other disciplines as needed
6. Assists with patient education for patient/family
7. Reviews lab results timely and notifies nephrologist when appropriate
8. Organizes and prioritizes work to utilize resources wisely, and complete assignments timely minimizing overtime
9. Demonstrates flexibility in response to unexpected changes to work volume and notifies Regional Directors when unable to complete work
10. Assists with the development of departmental policies and procedures
11. Demonstrates leadership and sensitivity toward co-workers by assisting as necessary and sharing skills and experience in a team environment

13. Promotes positive morale
14. Addresses systems problems/concerns with practical solutions
15. Provides for patient safety needs as indicated by identified risk factors and reports all concerns to Regional Directors.

Anemia:
1. Implement anemia management protocols determined by Medical Director and Nephrologist(s); review parameters and laboratory results to evaluate effectiveness of the Anemia Management Program
2. Review new patients’ admission labs and initiate anemia management protocols as appropriate and as directed by Nephrologists
3. Review all Hgb / Ferritin / T-Sat results and recommend adjustments to ESA and iron doses per established protocols for all initial and continued therapies
4. Maintain proactive communication with facility RN’s regarding patient specific conditions, events, etc. related to the Anemia Management Program
5. Train, coach, and mentor new RNs on basics of anemia management and anemia management protocols
6. Assist in the education of patients when necessary on anemia and iron management as it relates to their personal care and specific medical needs
7. Communicate with hospital acute programs when necessary to ensure continuation of ESA and iron therapies during hospitalizations
8. Attend monthly corporate QAPI meetings and report outliers

Access Management:
1. Review patient accesses monthly and make recommendations to Nephrologists to convert CVC to AVF when appropriate.
2. Document in EMR all information related to access management.
3. Work in collaboration with Concerto RN’s to initiate vein mapping, access revision, etc.
4. Report catheter rates to the Renal Network

QAPI:
1. Gathers all information for QAPI on a monthly basis
2. Enters data into the monthly minutes
3. Collaborates with Regional Directors of Operations on data and action plans needed
4. Assists in on-going evaluation of all tools and documentation related to QAPI
5. Reports monthly to the Corporate QAPI
